Pre-Qualifying Question:
We have carefully read the information provided about the Summer Youth Program Fund. The board of directors has approved the submission of this proposal.  If we receive a grant, we agree to use the funds as granted and will submit an evaluation report on or before Friday, September 26, 2008.  We will allow the funders to use information provided in this request for public information pieces and will mention each funder’s support in any publicity generated regarding the project. I accept the above statements by having a board member or executive director of the organization check the certification statement in our organizational profile.

Organizational Information

Summer Program Activities
Describe program components and daily activities.  Include: Program Activity, Percentage of Time and Specific Activities and how often they will occur. Be specific! Example: Remedial Education, 10% of time, Remedial math and writing classes 3 times per week for 2 hours/day *Content contains health and fitness information
Show the total number of administrative personnel who will staff the summer program.  Be sure to indicate how many are summer-only staff and how many are existing organization staff.

Show the total number of youth staff stipends who will staff the summer program.  Be sure to indicate how many are summer-only staff and how many are existing organization staff.

Show the total number of unpaid volunteers who will staff the summer program.  Be sure to indicate how many are summer-only staff and how many are existing organization staff.

Indicate the expected number of paid summer program staff and volunteers who have less than a high school education.

Indicate the expected number of paid summer program staff and volunteers who are high school graduates.

Indicate the expected number of paid summer program staff and volunteers who have a Bachelors Degree.

Indicate the expected number of paid summer program staff and volunteers who have a Graduate Degree.

Ratio of daily volunteers and/or paid program staff to daily number of participating youth.

Summer Program Evaluation
Clearly list your summer program's expected outcomes and evaluation methods.  Outcomes should be specific and measurable. Example:  Outcome: 40 students will become better problem solvers. Eval. Method: Instructor monitoring and observations.

How will youth, parents and community groups be involved in your evaluation?

Funders Checklist

The following funders within the SYPF collaborative will only fund an organization one grant per calendar year.  If you are concerned about a grant submitted directly to a particular funder, you are able to request these funders not review your SYPF request.  Please check the funder(s) that you do NOT want to consider your SYPF request.
